اَللّٰهُمَّ صَلِّ عَلَى سَيِّدِنَا مُحَمَّدٍ النَّبِيِّ الْأُمِّيِّ وَعَلَى آلِهِ وَصَحْبِهِ وَسَلِّمْ
Final recitation:
اَللّٰهُمَّ صَلِّ عَلَى سَيِّدِنَا مُحَمَّدِنِ الَّذِي جَاءَ بِحَقِّ الْمُبِينِ وَأَرْسَلْتَهُ رَحْمَةً لِلْعَالَمِينَ
Allāhumma ṣalli ʿalā sayyidinā Muḥammadin an-nabiyyi l-ummiyyi wa ʿalā ālihi wa ṣaḥbihi wa sallim.
Final recitation:
Allāhumma ṣalli ʿalā sayyidinā Muḥammadini lladhī jāʾa bi-ḥaqqi l-mubīn wa arsaltahu raḥmatan li-l-ʿālamīn.
O Allah! Send blessings and peace upon our master Muhammad, the unlettered Prophet, and upon his family and his Companions.
Final recitation:
O Allah! Let blessings and peace be upon our master Muhammad, whom You sent as a mercy to the worlds. He brought us the clear truth.

Salat al-Ummiyyah consists of words of dua for the Prophet (peace and blessings be upon him), for the members of his family, and for those close to him, and it is recited after every four rakahs in the Tarawih salah (prayer).
The Tarawih salah is a voluntary (nafilah) prayer particular to the month of Ramadan. The Prophet (peace and blessings be upon him) led it in congregation on some nights, then discontinued it "out of fear that it would become obligatory" (Sahih al-Bukhari, Tarawih 1). It began to be prayed in congregation in the time of Umar (may Allah be pleased with him).
This salawat is recited after every four rakahs, and a short rest is taken. In the final repetition a different wording is used: the phrase "You sent him as a mercy to the worlds" is added.
The Tarawih salah is prayed as 20 rakahs (in the Hanafi school). Salat al-Ummiyyah is recited after every 4 rakahs, so it is repeated a total of 5 times.
O Allah! Send blessings and peace upon our master Muhammad, the unlettered Prophet, and upon his family and his Companions. You sent him as a mercy to the worlds. Grant us his intercession, O Lord. Amin.
