سَجَدَ وَجْهِيَ لِلَّذِي خَلَقَهُ وَصَوَّرَهُ وَشَقَّ سَمْعَهُ وَبَصَرَهُ بِحَوْلِهِ وَقُوَّتِهِ فَتَبَارَكَ اللّٰهُ أَحْسَنُ الْخَالِقِينَ
On Rising from the Prostration:
سَمِعْنَا وَأَطَعْنَا غُفْرَانَكَ رَبَّنَا وَإِلَيْكَ الْمَصِيرُ
During the prostration: Sajada wajhiya li-lladhī khalaqahū wa ṣawwarahū wa shaqqa samʿahū wa baṣarahū bi-ḥawlihī wa quwwatih. Fa-tabāraka llāhu aḥsanu l-khāliqīn.
On rising from the prostration: Samiʿnā wa aṭaʿnā ghufrānaka Rabbanā wa ilayka l-maṣīr.
During the prostration: My face has prostrated to the One who created it and gave it form, and who opened its hearing and its sight by His might and power. How exalted is Allah, the best of creators.
On rising from the prostration: We have heard and we have obeyed. Our Lord! We ask Your forgiveness. To You alone is the final return.

Narrated by Aishah (may Allah be pleased with her). When our Prophet (peace and blessings be upon him) performed the prostration of recitation in the night prayer, he would recite this dua. (Sahih Muslim, Musafirin 771; Jami' at-Tirmidhi, Salat; Sunan Abi Dawud, Salat)
Our mother Aishah (may Allah be pleased with her) relates: "When the Messenger of Allah (peace and blessings be upon him) was reciting the Qur'an in the night prayer and came to a verse of prostration, he would prostrate and recite this dua" (Sahih Muslim, Musafirin 771). The prostration of recitation is a prostration that must be performed by anyone who recites or hears one of the fourteen verses of prostration in the Noble Qur'an. According to the Hanafis the prostration of recitation is obligatory (wajib), while according to the Shafi'is and Hanbalis it is an emphasised sunnah.
In performing the prostration of recitation, the servant demonstrates in deed that he obeys the command of the Noble Qur'an. In the verses of prostration Allah Most High commanded prostration to Him or praised those who prostrate. That the servant falls into prostration the moment he hears these verses is the finest sign of his attachment to the Qur'an and his obedience to Allah.
The phrase "Sami'na wa ata'na" (We have heard and we have obeyed), recited on rising from the prostration, is taken from the 285th ayah of Surah al-Baqarah. This phrase declares that the moment the believer hears his Lord's commands he obeys them at once. If the same verse of prostration is recited more than once in a single gathering, performing one prostration is sufficient.
My Lord, my face prostrates to You. You created me, You gave me form, You gave me hearing and sight. All these blessings came about by Your might and power. I have heard Your Qur'an and I have obeyed. I ask Your forgiveness; my return is to You. Make me one of the servants who bow to the command of Your Qur'an and fall down before You as soon as they hear the verses of prostration, O Lord. Amin.
