رَبِّ أَوْزِعْنِي أَنْ أَشْكُرَ نِعْمَتَكَ الَّتِي أَنْعَمْتَ عَلَيَّ وَعَلَى وَالِدَيَّ وَأَنْ أَعْمَلَ صَالِحًا تَرْضَاهُ وَأَصْلِحْ لِي فِي ذُرِّيَّتِي إِنِّي تُبْتُ إِلَيْكَ وَإِنِّي مِنَ الْمُسْلِمِينَ
Rabbi awziʿnī an ashkura niʿmataka'llatī anʿamta ʿalayya wa ʿalā wālidayya wa an aʿmala ṣāliḥan tarḍāhu wa aṣliḥ lī fī dhurriyyatī innī tubtu ilayka wa innī mina'l-muslimīn.
My Lord! Inspire me to give thanks for the favours You have bestowed upon me and upon my parents. Grant me to do the righteous deeds with which You are pleased. And set right my offspring for me. Truly I have repented to You, and truly I am among the Muslims.

It is the dua that the Noble Qur'an counsels a person to make on reaching forty, the age of maturity.
Allah Almighty said: "When a person reaches the age of full strength (forty), he says: 'My Lord! Inspire me to give thanks for the favours You have bestowed upon me and upon my parents, and grant me to do the righteous deeds with which You are pleased. And set right my offspring for me. I have repented to You, and I am among the Muslims'" (al-Ahqaf 15).
Forty is a special turning point, described in the Qur'an as "ashudd" (the age of maturity). When a person reaches this age they have spent half of life, gained experience and matured. Revelation also came to most of our prophets at the age of forty.
This dua contains five requests: giving thanks for blessings (including the blessing of one's parents), doing righteous deeds, the setting right of one's offspring (that one's children be righteous), repentance, and being among the Muslims. This dua is valid not only for those who are forty, but for every believer of every age.
The part "Aṣliḥ lī fī dhurriyyatī" (set right my offspring for me) is a request that one's children be righteous and on the straight path. The greatest dua of a mother and father is that their children become good Muslims.
My Lord! Grant me to give thanks for the favours You have given to me and to my parents. Inspire me to do the righteous deeds with which You are pleased. Set right my offspring and my children. Raise them as good Muslims. I have repented to You. Make me one of the Muslims, O Lord. Amin.
