اَللّٰهُمَّ إِنِّي أَسْأَلُكَ الصِّحَّةَ وَالْعِفَّةَ وَالْأَمَانَةَ وَحُسْنَ الْخُلُقِ وَالرِّضَا بِالْقَدَرِ
Another narration:
اَللّٰهُمَّ أَصْلِحْ لِي دِينِيَ الَّذِي هُوَ عِصْمَةُ أَمْرِي وَأَصْلِحْ لِي دُنْيَايَ الَّتِي فِيهَا مَعَاشِي وَأَصْلِحْ لِي آخِرَتِي الَّتِي فِيهَا مَعَادِي وَاجْعَلِ الْحَيَاةَ زِيَادَةً لِي فِي كُلِّ خَيْرٍ وَاجْعَلِ الْمَوْتَ رَاحَةً لِي مِنْ كُلِّ شَرٍّ
Allāhumma innī asʾaluka'ṣ-ṣiḥḥata wa'l-ʿiffata wa'l-amānata wa ḥusna'l-khuluqi wa'r-riḍā bi'l-qadar.
O Allah! I ask You for health, chastity, trustworthiness, good character and contentment with the decree.

Narrated by Abu Hurayrah (may Allah be pleased with him). (Musnad Ahmad II/374/22442; al-Tabarani, al-Mu'jam al-Awsat)
Abu Hurayrah (may Allah be pleased with him) relates: "The Messenger of Allah (peace and blessings be upon him) used to make this dua: 'O Allah! I ask You for health, chastity, trustworthiness, good character and contentment with the decree'" (Musnad Ahmad II/374).
These five requests cover the believer's happiness both in this world and in the Hereafter:
Health (sihhah): A healthy body is the first condition for being able to worship Allah. There is reward in illness too, but health and well-being are the greatest of blessings. The Prophet said, "There are two blessings in which many people are deceived: health and free time" (Sahih al-Bukhari, Riqaq 1).
Chastity (iffah): To keep away from what is unlawful and to remain within the lawful. It is to protect the eye, the tongue and the other limbs from the unlawful.
Trustworthiness (amanah): To keep one's word and not to betray a trust. A person must be reliable.
Good character: To get on well with people, to be courteous, to be merciful. The Prophet said, "The best of you are those who are best in character" (Sahih al-Bukhari, Adab 39).
Contentment with the decree: To be pleased with what Allah has determined. Whether it is good or bad, it is to know that it comes from Allah and to accept it.
O Allah! Grant me health and well-being. Make me chaste and keep me far from what is unlawful. Make me one who guards what is entrusted to him. Give me good character and let me show love to people. Grant me to be pleased with Your decree and to submit to what You determine, O my Lord. Amin.
