اَللّٰهُمَّ اغْفِرْ لِ(فُلَانٍ) وَارْفَعْ دَرَجَتَهُ فِي الْمَهْدِيِّينَ وَاخْلُفْهُ فِي عَقِبِهِ فِي الْغَابِرِينَ وَاغْفِرْ لَنَا وَلَهُ يَا رَبَّ الْعَالَمِينَ وَافْسَحْ لَهُ فِي قَبْرِهِ وَنَوِّرْ لَهُ فِيهِ
Allāhumma ghfir li-(fulānin) wa-rfaʿ darajatahu fi-l-mahdiyyīna wa-khlufhu fī ʿaqibihi fi-l-ghābirīn. Wa-ghfir lanā wa-lahu yā Rabba al-ʿālamīn. Wa-fsaḥ lahu fī qabrihi wa-nawwir lahu fīh.
O Allah! Forgive (so-and-so). Raise his rank to the level of those who are rightly guided. Be a guardian for those he has left behind. O Lord of the worlds! Forgive us and forgive him. Widen his grave and fill it with light.

Narrated by Umm Salamah (may Allah be pleased with her). When Abu Salamah (may Allah be pleased with him) passed away, the Messenger of Allah (peace and blessings be upon him) went in to him, closed his eyes which had remained open, and recited this dua. (Sahih Muslim, Jana'iz 7/920)
Umm Salamah (may Allah be pleased with her) relates: "The Messenger of Allah (peace and blessings be upon him) went in to Abu Salamah, who had passed away. His eyes had remained open, and he closed them. Then he said, 'When the soul departs, the eyes follow it'" (Sahih Muslim, Jana'iz 7). The eyes becoming fixed on a point after death is a natural condition that occurs during the departure of the soul.
When some members of Abu Salamah's family began to weep and wail aloud, our Prophet (peace and blessings be upon him) said: "Pray for yourselves with nothing but good, for the angels say amin to your prayers." Then he recited this dua (Sahih Muslim, Jana'iz 7). There are four important requests in this dua: asking for forgiveness, the raising of the rank, Allah's becoming a guardian for those left behind, and the grave being widened and filled with light.
When a Muslim passes away, the first thing done is to close the eyes and to bind the jaw from over the head. That the Prophet performed this himself has become a practical sunnah for the community. Those standing by the deceased should make good dua instead of lamenting, because at that moment the angels are saying "Amin."
O Allah, forgive our brother who has passed away. Raise his rank among the people of guidance. Take care Yourself of those he has left behind. O Lord of the worlds, encompass both us and him with Your forgiveness. Widen his grave and fill it with light. Make him steadfast in the face of the questions of Munkar and Nakir, O Lord. Amin.
