While breaking the fast:
اَللّٰهُمَّ لَكَ صُمْتُ وَبِكَ آمَنْتُ وَعَلَيْكَ تَوَكَّلْتُ وَعَلَى رِزْقِكَ أَفْطَرْتُ
After breaking the fast:
ذَهَبَ الظَّمَأُ وَابْتَلَّتِ الْعُرُوقُ وَثَبَتَ الْأَجْرُ إِنْ شَاءَ اللّٰهُ
At iftar: Allāhumma laka ṣumtu wa-bika āmantu wa-ʿalayka tawakkaltu wa-ʿalā rizqika afṭartu.
After iftar: Dhahaba aẓ-ẓamaʾu wa-btallati al-ʿurūqu wa-thabata al-ajru in shāʾa Allāh.
At iftar: O Allah! I fasted for Your good pleasure, I believed in You, I put my trust in You, and I break my fast with the provision You have given.
After iftar: The thirst has gone, the veins are moistened, and, if Allah wills, the reward has been confirmed.

Narrated by Ibn Umar (may Allah be pleased with him). Our Prophet (peace and blessings be upon him) would recite these duas when he broke his fast. (Sunan Abi Dawud, Sawm 22; al-Daraqutni, Siyam II, 185)
Ibn Umar (may Allah be pleased with him) relates: "After the Prophet (peace and blessings be upon him) broke his fast he would say, 'The thirst has gone, the veins are moistened, and, if Allah wills, the reward has been confirmed'" (Sunan Abi Dawud, Sawm 22). Our Prophet would also pray while breaking his fast: "O Allah! I fasted for Your good pleasure and I break my fast with the provision You have given" (Sunan Abi Dawud, Sawm 22).
Anas ibn Malik (may Allah be pleased with him) relates: "The Messenger of Allah (peace and blessings be upon him) would break his fast with a few fresh dates before praying the maghrib salah (prayer); if there were none, with dried dates. If there were none of those either, he would break his fast with a few sips of water" (Sunan Abi Dawud, Sawm 21; Jami' at-Tirmidhi, Sawm 10). This hadith shows that our Prophet's iftar table was simple and that, without delay, he would first break his fast and then perform the salah.
The moment of iftar is one of the blessed times at which dua is accepted. Our Prophet (peace and blessings be upon him) said: "The dua of three people is not rejected: the dua of the fasting person at the time of iftar, the dua of a just head of state, and the dua of the oppressed" (Jami' at-Tirmidhi, Da'awat 129). For this reason it is recommended (mustahabb) to make abundant dua at the moment of iftar and to lay one's needs before Allah.
O Allah, I fasted for Your good pleasure and I break my fast with the provision You have given. Accept my fast. You have taken away the thirst, You have moistened the veins, and, if Allah wills, You have confirmed the reward. Make this day I have spent fasting acceptable. Make me among Your servants who keep the fast as it should be kept, who give thanks at iftar, and who take their share of the blessing of Ramadan, O Lord. Amin.
