The intention is made in the heart.
While washing the hands:
بِسْمِ اللّٰهِ وَالْحَمْدُ لِلّٰهِ عَلَى الْإِسْلَامِ وَعَلَى تَوْفِيقِ الْإِيمَانِ
While rinsing the mouth:
اَللّٰهُمَّ أَعِنِّي عَلَى تِلَاوَةِ كِتَابِكَ وَكَثْرَةِ ذِكْرِكَ
While rinsing the nose:
اَللّٰهُمَّ أَرِحْنِي رِيحَ الْجَنَّةِ وَلَا تُرِحْنِي رِيحَ النَّارِ
While washing the face:
اَللّٰهُمَّ بَيِّضْ وَجْهِي يَوْمَ تَبْيَاضُ الْوُجُوهِ وَتَسْوَادُ الْوُجُوهِ
While washing the right arm:
اَللّٰهُمَّ أَعْطِنِي كِتَابِي بِيَمِينِي
While washing the left arm:
اَللّٰهُمَّ لَا تُعْطِنِي كِتَابِي بِشِمَالِي
While wiping the head:
اَللّٰهُمَّ حَرِّمْ شَعْرِي وَبَشَرِي عَلَى النَّارِ
While washing the ears:
اَللّٰهُمَّ اجْعَلْنِي مِنَ الَّذِينَ يَسْتَمِعُونَ الْقَوْلَ فَيَتَّبِعُونَ أَحْسَنَهُ
While washing the feet:
اَللّٰهُمَّ ثَبِّتْ قَدَمَيَّ عَلَى الصِّرَاطِ يَوْمَ تَزِلُّ الْأَقْدَامُ
After completing the wudu:
أَشْهَدُ أَنْ لَا إِلٰهَ إِلَّا اللّٰهُ وَحْدَهُ لَا شَرِيكَ لَهُ وَأَشْهَدُ أَنَّ مُحَمَّدًا عَبْدُهُ وَرَسُولُهُ ﴿﴾ اَللّٰهُمَّ اجْعَلْنِي مِنَ التَّوَّابِينَ وَاجْعَلْنِي مِنَ الْمُتَطَهِّرِينَ
After wudu: Ashhadu an lā ilāha illā llāhu waḥdahū lā sharīka lah, wa-ashhadu anna Muḥammadan ʿabduhū wa-rasūluh. Allāhumma ijʿalnī mina t-tawwābīna wa-jʿalnī mina l-mutaṭahhirīn.
I bear witness that there is no deity but Allah alone; He has no partner. And I bear witness that Muhammad is His servant and His messenger. O Allah, make me one of those who turn to You in repentance, and make me one of those who purify themselves.

Narrated by Uqbah ibn Amir (may Allah be pleased with him). The Messenger of Allah (peace and blessings be upon him) said: "Whoever performs wudu well and then looks to the sky and says, 'Ashhadu an lā ilāha illā llāh...', the eight gates of Paradise are opened for him and he may enter through whichever of them he wishes." (Sunan Ibn Majah, Taharah 57; Sahih Muslim, Taharah 38)
Uqbah ibn Amir (may Allah be pleased with him) relates that the Messenger of Allah (peace and blessings be upon him) said: "For whoever performs wudu well and then says, 'Ashhadu an lā ilāha illā llāh...', the eight gates of Paradise are opened, and he enters through whichever he wishes" (Sahih Muslim, Taharah 38). Wudu is the key to salah (prayer) and half of worship. As long as a person's wudu remains unbroken, the angels seek forgiveness for him.
The duas recited during wudu allow the worshipper, as each limb is washed, to voice a request concerning the hereafter connected with that limb. As the face is washed, one asks that the face be made radiant on the Day of Judgement; as the hands are washed, that the record of deeds be given in the right hand; as the feet are washed, that one cross the Sirat safe and sound. This is a beautiful sunnah that unites the worldly and the otherworldly dimensions of worship.
The Messenger of Allah (peace and blessings be upon him) said, "Wudu upon wudu is light upon light" (al-Bayhaqi, Shu'ab al-Iman). Wudu is an important act of worship, repeated five times a day, that provides the servant with physical cleanliness alongside spiritual purity.
O Allah, I perform my wudu seeking Your pleasure. With this water I have cleansed my outward self; cleanse my heart too with the light of faith. Forgive the faults my hands have committed, the wrongs my eyes have looked upon, and the false paths my feet have walked. On the Day of Judgement make my face radiant, give me my record of deeds in my right hand, and keep my feet firm upon the Sirat. Make me one of Your servants who purify themselves and turn to You in repentance, O Lord. Amin.
